Not reporting errors is only human

We all make mistakes sometimes. When problems arise, there is an opportunity to identify risks and minimize future mistakes and accidents. Professor Tahira Probst’s research shows that more than half of the employees she studied did not report accidents, partly because they did not see that reporting made any difference. Many also felt that accident reporting could have negative consequences, such as that the group or the employee would be blamed for the accident.
